- comments: observed over 30 mice droppings scattered on floor along wall near hand washing sink in east classroom, on floor along wall and corner under utility sink and on electrical outlets in kitchen. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed single service utensils not inverted. Instructed manager to invert all single service articles to protect from contamination.

- comments: observed ice buildup inside infant room small refrigerator. Instructed manager to defrost and maintain.
