- comments: 6-301. 12 observed no hand drying provision at the hand washing sinks located in the dish washing area. Instructed to always provide a hand drying provision at all hand washing sinks at all times. Priority foundation. Citation issued 7-38-030(c).

- comments: 4-602. 11(a) observed a black substance in the interior of the ice machine located in the dish washing area. Instructed to clean and sanitize. Priority violation. Citation issued 7-38-005.

- comments: 7-201. 11 observed toxic items stored next to food contact surfaces in the dish washing area. Instructed to remove and only store toxic substances in a separate storage area at all times. Priority violation. Citation issued 7-38-005.

- comments: 7-206. 11 observed a bottle of 'fly/gnat' spray on top of the ice machine located in the dish washing area. Instructed only pest control can handle all pests issues, facility may not use additional pesticides. Priority violation. Citation issued 7-38-015.

- comments: 2-402. 11 observed male food handlers with beards and without beard guards. Instructed to provide.

- comments: 4-904. 11 observed tables being preset without adequate protection for tableware. Instructed to not preset the customer dining tables or set tables upon customer arrival or invert plates, bowls and glasses as well as provide adequate protection for the chop sticks and spoons.

- comments: 4-501. 17 observed dish machine without adequate detergent. At the time of the inspection the detergent bottle was emptied. Instructed to provide detergent at the dish machine at all times. Priority foundation. Citation issued 7-38-025.

- comments: 5-103. 12 observed no cold water at the far-right hand washing sink located at the front food prep area. Instructed to repair and provide adequate cold running water at all sinks at all times. Priority violation. Citation issued under violation above.

- comments: 5-103. 11(b) observed inadequate hot water at the hand washing sinks and the 3 compartment sink, hot water only reached 85f at the time of the inspection. Instructed the facility must have adequate hot water at all sinks at all times, hot water must reach 100f at the hand washing sinks and 110f at the 3 compartment sink at all times. Priority violation. Citation issued 7-38-030(c).

- comments: 8-404. 13(b:3) observed previous priority foundation violation still not corrected from inspection report #2574682 dated 4/20/23; 8-404. 13 - at the time of the re-inspection observed about 20 live small flies on the ceiling of the dish washing area. Instructed to eliminate live small flies activity, clean and sanitize affected areas and recommended contacting pest control for a service. At the time of the re-inspection on 4/26/23 live small flies activity increased and the facility still had about 40 live small flies on the ceilings and walls in the dishwashing/food prep area. Instructed to eliminate live small flies activity, clean and sanitize affected areas and facility must contact pest control for a service. Priority violation. Citation issued 7-42-090.
