- comments: observed exposed handwashing sink in smoothie bar cannot be turned on due to broken hot water handle. Instructed to repair or replace and maintain all handwashing sinks fully supplied and capable of delivering running water between 100-120f. Priority violation 7-38-030(c). Citation issued.

- comments: observed no thermometer inside several prep coolers in the rear food prep area. Instructed to provide.

- comments: observed small gap along side of rear door leading to alley. Instructed to correct and maintain tight-fitting door closure.

- comments: observed no backflow preventer connected to the ice machine. Instructed to install following all relevant laws, rules, and regulations.

- comments: observed used cooking oil container in rear alley covered in heavy build up of grease and grime. Instructed to detail clean and maintain in good condition.

- comments: observed heavy build-up of grease, grime, food debris, and dirt on floors, walls, and equipment along the main cook line. Instructed to detail clean and maintain in good condition.

- comments: observed no allergen training certificates on site at the time of inspection. Instructed all employees holding city of chicago foodservice manager certificate must complete allergen training and maintain records thereof on site at all times.
