- comments: observed facility food handlers preparing and handling�time and temperature control for safety(tcs) foods without original city of chicago food service manager and certificate on site. Informed person in charge city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving and operating hours. Certified manager arrived after inspection was completed. Priority foundation violation #7-38-012. Citation issued.
36. Thermometers provided & accurate
- comments: noted no metal stem thermometer on site to monitor the internal food temperature of foods the facility may receive during deliveries (i. E. Milk, creamer, and hot burritos etc. ). Facility must obtain and maintain proper thermometers on site. Priority foundation violation #7-38-005. Citation issued
38. Insects, rodents, & animals not present
- comments: observed a prohibited animal (dog) on site in the facility. The person in charge was made aware that such animals are prohibited unless they are service animals. Instructed to move to outdoor patio area and remove animal from the facility and maintain at all times. Dog was removed during inspection. Priority foundation #7-40-060. Citation issued.

- comments: observed milk crates being used as shelving throughout the prep and storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: must provide drainboard, for the air drying of all utensils/dishware, in the kitchen.
