- comments: observed some bulk food containers being stored underneath preptables with no labels. Instructed to provide labels for bulk food containers containing items not in their original packaging.
39. Contamination prevented during food preparation, storage & display
- comments: observed plates and other utensils being stored facing up on shelves in kitchen preparation area. Instructed to invert all dishes and utensils to prevent contamination.
49. Non-food/food contact surfaces clean
- comments: observed accumulated grease and food debris exterior surfaces of equipment, prep tables,coolers, and shelving units. Instructed manager to clean and maintain. (nonfood contact surfaces)
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered trash receptacle in the womens toilet room. Instructed to provide a covered trash receptacle and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed dirty floors with food debris spills throughout kitchen preparation area. Instructed manager to detail clean and maintain

- comments: observed expired allergen awareness certificates. Instructed to renew expired allergen awareness certifications for all individuals that have a city of chicago food sanitation certificate.
