1. Person in charge present, demonstrates knowledge, and performs duties
- comments: pic did not have a valid city of chicago sanitation certificate.

2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago certified manager on duty/site while tcs foods are being prepared, handled and served such as chicken and fish. Also no valid city sanitation certificate posted. Instructed manager to enroll more employee's in the city sanitation class. Must be able to cover all shifts. Priority foundation citation issued #7-38-012.

55. Physical facilities installed, maintained & clean
- comments: floors need cleaning through out premises to remove spills,debris and grease.

56. Adequate ventilation & lighting; designated areas used
- comments: observed the filter above the cooking equipment dirty. Must clean and maintain.

57. All food employees have food handler training
- comments: instructed manager to comply with the idph food handler requirements or citations will follow.
