- comments: observed outside dumpsters with lids not closed due to trash overflowing. Also noted piles of trash, boxes and grease outside establishemnts rear door. Instructed to correct and maintain. Priority foundation violation #7-38-020(b). Citation issued.

- comments: noted trash and debris on the grounds surrounding enclosed dumpsters. Instructed to remove all trash and debris, clean and sanitize area and maintain.

- comments: noted grease receptacle and two enclosed dumpsters with encrusted food debris. Instructed to clean, sanitize and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed clutter in the office located in kitchen, rear dry storage and bathroom. Also noted unused equipment with encrusted grease in rear prep area and between hot water heater and 3 compartment sink. Instructed to remove all unused and unnecessary items to business. All kept items are to be stored on proper shelving that is non-porous, smooth and easy to clean and a minimum of 6 inches off the ground.
