- comments: observed an incomplete pest control logbook is missing one service report and the idph license number. Observed two service reports dated on 11/27/2024 and no idph license for new pest control company. Must provide and maintain all required pest control records and documents. - comments: observed the interior of the ice machine with minmial build up. Instructed to clean and maintain.
51. Plumbing installed; proper backflow devices
- comments: observed two drain pipes: one from the ice machine and another from the ice crusher machine directly connected pipe connected to the grease trap under the three compartment sink. Must provide indirect connection by providing an air gap to prevent cross-contamination. Priorty violation 7-38-030(c), citation issued
