- comments: observed no city of chicago certified food manager on site during the inspection while the time/temperature control for safety foods (cold sandwiches) has been prepared and served. Instructed a city of chicago certified food manager must be on site at all times with the original city of chicago food service manager certificate while open and operating. Priority foundation violation 7-38-012, citation issued

- comments: observed no proof of employee health policy on site. Instructed to provide an employee health policy that includes a signed acknowledgement from each employee. Priority foundation 7-38-010, no citation issued

- comments: observed no clean-up kit and procedure for clean up for vomiting and diarrhea on site. Instructed to have said procedure along with required equipment for clean-up. Priority foundation violation 7-38-005, no citation issued.

- comments: observed no exposed handwashing sink in the rear dishwashing area. Instructed to install a handwashing sink to allow convenient use by employees in all food preparation areas, food dispensing, and warewashing areas. The handwashing sink must meet compliance by having water from an approved source by the city of chicago, have hot and cold running water (100-120 f), and fully equipped with handwashing cleanser and hand drying provisions. Priority foundation 7-38-030(c), no citation issued.

- comments: observed no handwashing signage at the exposed handwashing sink in the front food prep area and in the employee toilet room. Insturcted to provide and maintain.

- comments: observed ready to eat, temperature control for safety foods (4 whole deli meats that has been sliced and 4 whole cheeses that have been sliced) not properly dated, stored longer than 24 hours in coolers. Instructed to provide proper date marking with disposition date for rte foods prepared on site and held over 24 hours in cold holding unit. Priority foundation violation 7-38-005, no citation issued.

- comments: observed no labeling on the dry ingredient storage container in the food prep area. Instructed to provide proper labeling on any food items not in its original packaging and maintain.

- comments: observed no pest control logbook on site with the required information such as: record of visits, copy of service ticket/license that sets forth pest control idph license number. Must provide and maintain all required pest control records and documents on site at all times. Priority foundation violation 7-38-020 (c), citation issued

- comments: observed a freezer unit with foods that are used for to go orders inside, stored outside the door of the food prep area, where customers can access the food. Instructed relocate said freezer unit or food items and maintain.

- comments: observed the interioir of the deli meat slicer not clean. Instructed to clean said equipment and maintain.

- comments: observed the door on the employee toilet room not self-closing. Instructed to install a self-closing device on the said door.

- comments: observed the storage closet near the entrance to the rear dishwashing area with unnecessary items and clutter. Instructed to remove all items that are not necessary to the daily operation and organize clutter and maintain.

- comments: observed one blown out light bulb under the ventilation system. Instructed to replace and maintain.
