- comments: observed no handwashing signage in the women employee washroom. Instructed to provide.

- comments: observed facility without a licensed pest control log book/operator. Instructed to provide. Priority foundation violation. 7-38-020(c). Citation issued.

- comments: observed improper storage of wiping cloths on 3-compartment sink #1. Instructed to store properly.
44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed improper storage of cookware/kitchenware on shelving unit in the kitchen area; cookware not inverted. Instructed to store properly to prevent contamination.

- comments: observed ice build up on the condensor in the walk in freezer. Instructed to adjust/repair and maintain.

- comments: observed 1 of the three compartment sink and the four compartment sink not maintained; sinks are leaking when water from the compartments are drained. Instructed management to get drainage system serviced. Priority foundation 7-38-025. Citation issued.
