- comments: 4-501. 12 observed cutting boards with deep scratches and dark stains on prep tables in the front preparation area. Instructed the person in charge to resurface/replace cutting boards and maintain them.

- comments: 4-202. 16 observed rusted storage racks in the walk-in cooler and walk-in freezer. Instructed the person in charge to remove rust and maintain. All nonfood-contact surfaces must be smooth and easily cleanable.

- comments: 4-101. 17 observed raw wood shelves in the rear dry storage and the front and rear preparation areas. Instructed the person in charge to seal or paint to provide a smooth and easily cleanable surface.

- comments: 6-501. 11 observed missing ceiling tiles near the rear exit door. ---also observed multiple gaps in the ceiling in the rear preparation area. Instructed the person in charge to replace missing ceiling tiles smoothly to seal all gaps and maintain them in good repair.

Instructed the person in charge to detail clean under all shelving units, interior of prep coolers, hot-holding units, prep tables, and deep fryers to remove spills, food debris, dirt buildup, and or grease. Must maintain at all times.

- comments: 2-102. 13 observed one employee with an expired food handler certificate. Instructed the person in charge to renew the expired food handler certificate to comply with idph food handler requirements.
