- comments: observed open cups of beverages on prep table. Instructed manager to provide closed lid containers for employee beverages to prevent contamination.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed several large bulk container lids stored inside hand washing sink in prep area. Instructed manager to remove all items from hand washing sink to make accessible for hand washing at all times. Priority foundation 7-38-030(c) citation issued

16. Food-contact surfaces: cleaned & sanitized
- comments: observed low temperature dish machine not sanitizing food equipment and utensils, chemical ppm 0 instructed manager to repair and maintain 100 ppm. Dish machine is tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-025 citation issued

38. Insects, rodents, & animals not present
- comments: observed over 100 mice droppings scattered on floor under shelving units in prep area, along walls and corners on floor behind coolers, on floor in utility storage area, on bases of equipment in prep area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed soiled wiping cloths stored on prep tables. Instructed manager to store wiping cloths in sanitizing solution between uses.

- comments: observed single service articles not inverted in prep area and stored on floor in storage area. Instructed manager to invert and elevate all single service articles to protect from contamination.

- comments: observed rust deposits on bottom of prep tables, instructed manager to remove all rust deposits on equipment and maintain.

- comments: observed accumulated grease buildup and food debris on interior and exterior surfaces of cooking equipment, shelving units, holding units, prep tables, rolling carts, coolers and freezers. Instructed manager to clean and maintain.

- comments: observed leaking hose in rear storage area. Instructed manager to remove and maintain.
