10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing cleanser at the handwashing sink located in dishwashing/prep room. All handwashing sinks must be equipped with handwashing soap at all times. Instructed manager to provide. Priority foundation 7-38-030-(c). Management provided hand soap at said sink.

29. Compliance with variance/specialized process/haccp
- comments: observed sushi rice (vinegar added to rice) at 113f in the front prep area. Observed the facility without a sushi lab letter or a ph meter capable of determining the ph of the rice. Instructed management to obtain a ph meter or a sushi rice lab letter and maintain it on site. Priority foundation 7-38-005. Citation issued.

49. Non-food/food contact surfaces clean
- comments: found heavy grease build up inside the fryers and on the exterior parts of equipment on the cooks line. Found a black mold-like build-up on the dish machine trays. Must detail clean all and maintain.

55. Physical facilities installed, maintained & clean
- comments: found a thick layer of grease and dirt on the floor, underneath equipment, and near the wall base throughout the rear prep/dish room. Must detail clean floors and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed both lights above the cook's line burned out. Must replace burned-out light bulbs and provide adequate lighting in the rear prep/dish room.
