High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 15 flying insects on the walls and menu at the front counter. Observed 10 flying insects at the back of the restaurant. Observed 10 flying insects in the dining room condiment station flying and crawling on the ketchup dispenser pump.
High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Open bag of raw chicken patties above open bag of precooked sausage in the reach in freezer.
High Priority - Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands.
High Priority - Employee dried hands on clothes/apron/soiled towel after washing.
High Priority - Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands.
High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands.

Friday, April 12th, at around 10am I ordered a bacon egg and Gouda sandwich as well as a caramel ribbon drink. By around 3-4pm, I started feeling unwell. At 5 pm I vomited for the first time.
