- comments: observed some female food handlers without proper hair restraints such as a hat and hair net and hair not properly restrained. Instructed all food handlers must wear hair restraints such as a hat or hair net and properly restrain hair at all times while in the food prep area.
51. Plumbing installed; proper backflow devices
- comments: observed a minor water leak on a pipe located under the 3 compartment sink in the rear food prep area. Instructed to repair.
55. Physical facilities installed, maintained & clean
- comments: observed floors throughout with accumulated food, grease and some minor dirt debris. Instructed to clean under the 3 compartment sink, under the dry storage shelves in the rear and under/behind the ice machine, refrigeration units and cooking equipment.
57. All food employees have food handler training
- comments: observed a list of the food handlers that completed the food handlers course however the facility does not have an idph approval letter to verify that the facility's food handler training is approved by idph. Instructed to provide an idph approval letter for food handler training cirriculum.
58. Allergen training as required
- comments: the city of chicago food service sanitation manager that is on duty does not have proof of allergen training. Instructed to provide proof of allergen training or meet allergen training requirement.
