- comments: we observed over 40 small flies in different areas of the kitchen, ice machine area, dish room, and cold prep area. Instructed to remove said flies, clean and maintain. Priority foundation violation:7-38-020(a), citation issued

- comments: waste pipe in dish room wraps are broken, peeling tape, not cleanable, also above white reach-in freezer rear area. Must encase said pipes. Surface must be smooth, cleanable and non-absorbent surface

- comments: ice build-up at condenser unit inside the walk-in freezer. Repair issue.

- comments: broken rubber gasket inside the white reach-in freezer, also at two-door reach-in cooler in cold food preparation, also the same unit left door broken from hinges, not able to close completely. No handle was provided at white reach-in freezer. Broken and missing doors under the cabinet inside the women's staff washroom. Repair/replace.

- comments: some cutting boards in the kitchen with deep cuts, must refinish or replace said cutting boards

- comments: no irreversible registering temperature indicators for high-temperature dish machines. Instructed to provide an irreversible registering temperature indicator, such as strips/stickers or thermometer. Priority foundation violation: 7-38-005. No citation was issued.

- comments: additional hot water flow is needed at washbowls inside the women's washroom.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: no cover for receptacle inside the staff washroom, or replace broken individual receptacles inside each stall

- comments: broken east wall at mop sink. Broken wall and floor under the dish machine. Broken floor in front of prep/cook area. Missing and broken ceiling tiles at cold room. Caulking at handwashing sing in prep area is broken with black substance. The surface must be smooth, cleanable and non-absorbent surface

- comments: protective light shield in cold room with excess debris and food stains, clean and maintain

- comments: previous core violation not corrected from 8-26-2019, report #2311271 #55) 6-201. 11 observed floor drains in dish room and drain for dish machine need to be scrubbed and treated to prevent small fly breeding grounds. Clean and maintain. Priority foundation violation:7-42-090, no citation issued
