- comments: observed food employee at front prep area with poor hygienic practices, touching different items with gloved hands and performing multiple tasks, and then preparing customers' food orders without changing their gloves or washing hands. Instructed to always wash hands after engaging in activities that contaminate the hands. Priority violation. 7-38-010. Citation issued.

- comments: noted improper internal temperatures of the following time/temperature control for safety food items inside the food display cooler improper: chicken 46. 6 f to 47. 4 f. Manager immediately and voluntarily disposed of 3. 5 lbs of products worth $18. 00 through the denaturing process. Priority violation #7-38-005. Citation issued.
37. Food properly labeled; original container
- comments: allergen labels are missing on food packaged in retail food establishments. Observed allergen labels missing on pre-packaged foods in the customer grab-and-go cooler located near the deli service counter. Foods include pasta salad, chicken salad, sandwiches, and salads. Must label the name of the food sources of any major food allergens (i. E. , milk, eggs, wheat, soy, etc. ) on the containers and maintain. Priority foundation violation 7-38-005. Citation issued. Foods have been removed from self-service at the time of inspection.

- comments: observed previous core violations from the inspection #2578971 from 07/24/2023 as follows: 3-305. 12: **c** food storage, prohibited areas observed cases of wine stored inside the washroom. Instructed management that food may not be stored inside washrooms. Must relocate and maintain. 2-402. 11: **c** effectiveness- hair restraints observed food handling employees preparing food without hair restraints. Management instructed that all food-handling employees must wear a hair restraint while preparing food. 2-102. 13: **c** food handler training (pa 100-0367 food handling regulation enforcement act) observed food-handling employees without training. Instructed management that all food handlers must complete approved training and maintain certificates on site. 2-102. 13: **c** allergen training�all certified food service sanitation managers employed by a restaurant must receive or obtain training in basic allergen awareness principles within 30 days of employment and every three years thereafter. Training programs must be accredited by the american national standards institute or another reputable accreditation agency under the astm international e2659-09 (standard practice for certificate programs). Observed no certified food manager with allergen training. Instructed any persons with the city of chicago food sanitation certificate to comply with the completion of training. Must provides documentation. . Instructed manager to correct violations. Priority foundation violation # 7-42-090 citation issued.
