- comments: observed no certified manager on site between 8:20am and 8:50am while tcs items such as eggs were prepared. Certified manager shadik awwad arrived on site at 8:50am. Instructed to maintain an employee holding the city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no verifiable employee health policy on site at time of inspection. Left template and instructed to maintain copies of employee health policy signed by all employees on site at all times. Priority foundation violation 7-38-010. Citation issued.

- comments: observed no thermometer inside rear grab-and-go cooler. Instructed to provide.

- comments: observed deli salads sold in 'to-go' containers without labels. Instructed to provide disclosing ingredients and trace back information.

- comments: unable to located sink stoppers for 3 compartment sink. Instructed to provide.

- comments: observed no covered trash reeptacle inside unisex employee toilet room. Instructed to provide.

- comments: observed build up of food debris on deli slicers located in deli area. Instructed to detail clean and maintain in good condition.
57. All food employees have food handler training
- comments: observed no food handler training certificates on site at time of inspection. Instructed all food employees must complete food handler training and maintain records thereof on site at all times.
60. Previous core violation corrected
- comments: observed the following repeat core violation not corrected from report #2568037 dated 12/6/2022: #39 'observed hand sink located next to prep cooler and table at deli without a splash guard. Must install splash guard at hand sink. ' instructed to correct and come into compliance. Priority foundation violation 7-42-090. Citation issued.
