49. Non-food/food contact surfaces clean
- comments: observed accumulated grease and food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers, freezers, shelving units, grease trap, interior of all deep fryers, and light shields. Instructed manager to clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed several dead roaches on floor throughout facility. Must detail clean and maintain same. Observed a heavy accumulation of dirt, dust, grease, food splatter and food debris on the walls and floors especially along the hot food service line under and around the cooking equipment and non cooking equipment. Instructed to clean and maintain all areas. Observed debris on floors along walls and corners throughout facility. Instructed manager to clean and maintain.

59. Previous priority foundation violation corrected
- comments: previous priority foundation violation from report #2579366 on 8/1/23 not corrected: 38 - observed over 12 live cockroaches on the rear handsink, walls, front counter area, and around the dish washing area. Additional pest control service is needed to eliminate the pest activity throughout. Priority violation 7-42-090.
