- comments: observed the rear walk in freezer with ice build up. Must defrost clean and maintain.

51. Plumbing installed; proper backflow devices
- comments: observed one of the hand sinks in the customer(women) washroom not properly working. Also noted a leak at the dish wash sink area at the bottom. Must repair and maintain.

55. Physical facilities installed, maintained & clean
- comments: must clean the floors and wall around cooking equipment to remove debris and/or grease.
