- comments: no original city of chicago sanitation certificate on site at time of inspection.

- comments: no one on site at time of inspection with an original city of chicago sanitation certificate. Certificates may not be taken from photo on cell phone. Someone with an original city of chicago sanitation certificate must be on site when handling tcs food products such as chicken wings, chicken tenders, chicken sandwiches etc. . . Instructed person in charge to have an original valid city of chicago sanitation certificate at all times. Priority foundation 7-38-012 citation issued
9. No bare hand contact with rte food or a pre-approved alternative procedure properly allowed
- comments: observed employee handling ready to eat food products with bare hands. Food handler was cupping up ranch dressing with bare hands. Instructed to have gloves at all times when handling ready to eat foods. Dressing cups were discarded at this time. Total value 30 cups 15 lbs $50 priority violation 7-38-010 citation issued

- comments: observed rear prep sink with no soap or good working soap dispensor. Person in charge provided soap and corrected dispensor. Instructed to have soap and dispensor in good working order at all times. Priority foundation 7-38-030(c) citation issued

- comments: incomplete pest control logbook on site at time of inspection. Instructed to have pest control provide complete pest control logbook pest control license, service receipts etc. . . Priority foundation 7-38-020(c) citation issued

- comments: interior of ice machine in need of cleaning to remove build up. Instructed to clean and maintain.

- comments: all food handlers must wear hair restraints while handling food. Instructed to provide hair nets to have all hair restrained. Long hair must be tied up in hair nets.

- comments: interior of microwave oven in need of detail cleaning. Instructed to clean and maintain.
