- comments: observed over 20 small live flies on wall in rear mop room area. Instructed to remove, clean and sanitize the affected area. Recommend to call licensed pest control for service. Priority foundation violation 7-38-020 (a) citation issued

- comments: instructed to detail clean and maintain storage racks inside walk-in cooler.

- comments: unable to locate greasetrap for establishment's utensil washing sink. Must provide location or install grease trap.

Waste water backing up from both washroom floors, and floors in rear storage room, moproom and in rear office, also one handsink in front prep area completely clogged and the other handsink in middle prep area slowly draining. Instructed to repair and maintain. Priority violation 7-38-030 (c) citation issued.

Priority foundation violation 7-42-090 citation issued #38 - instructed to rodent proof bottom of rear exit door. #55 - instructed to detail clean and maintain floor under and behind ice machine in prep area and must detail clean and maintain floor drain with excessive grease and mold like accumulation underneath 3 compartment sink.
