2. City of chicago food service sanitation certificate
- comments: no chicago certified food manager nor valid certificate on site while preparing/serving tcs foods (i. E. Chicken). Instructed to have a cfm on site at all times when preparing/serving tcs foods. Priority foundation 7-38-012 citation issued
38. Insects, rodents, & animals not present
- comments: the rear exit door with a 1/4" gap underneath and is not rodent/insect proofed. Instructed to rodent/insect proof and maintain rear exit door.
40. Personal cleanliness
- comments: employees in the food prep area and handling food are not wearing hair nor beard restraints. Instructed to have all in the prep area and handling food to wear a hair/beard restraints. Maintain same.

- comments: tongs stored on top of the container inside the walk in cooler. Instructed to properly store tongs in a clean bin/container and maintain.

- comments: the wash bowls in the men/women's toilet room needs to be recaulked around the top surface. Instructed to recaulk around the wash bowls and maintain.

- comments: the top interior/exterior of the deep fryers with a thick build up of grease, food and dirt. Instructed to clean and maintain the interior/exterior of the deep fryers.
55. Physical facilities installed, maintained & clean
- comments: broken/cracked floor tiles in front of the fryers. Instructed to replace floor tiles and maintain a smooth and easily cleanable surface.
