- comments: observed several employees who did not sign the employee health policy. Management instructed that all employees must sign the employee health policy. Priority foundation 7-38-010.

- comments: clean the grease on the interior and exterior of the deep fryer.
55. Physical facilities installed, maintained & clean
- comments: clean the grease on the wall behind the cooking equipment and under the ventilation hood and in the corner of the dry storage area.
56. Adequate ventilation & lighting; designated areas used
- comments: observed one of the light fixtures with unshielded light bulbs above the food prep area. Management instructed to install plastic end caps on both light fixtures or a light shield which covers the whole fixture or install shatterproof light bulbs.
