10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand paper towels at the handwashing sinks located in the rear dish and the front drink prep area. All handwashing sinks must be equipped with a hand drying provision at all times; instructed to provide. Corrected on site. Priority foundation 7-38-030(c). Citation issued.

- comments: observed no soap at the handwashing sinks located in the rear dish and the front drink prep area. All handwashing sinks must be equipped with hand-cleaning liquid at all times. Instructed manager to provide. Priority foundation 7-38-030-(c). Combined with the above citation.

16. Food-contact surfaces: cleaned & sanitized
- comments: found low-temperature dish machine broken and unable to sanitize. The manager states kitchen currently only uses a 3-compartment sink, and ecolab has been contacted to come to remove the machine. Instructed to continue using the 3-comp sink to wash, rinse, and sanitize all dishes, pots, and pans until the machine has been serviced/ repaired. Priority violation 7-38-025. The dish machine is tagged; instructed to contact cdph for reinspection if the machine is repaired.

- comments: must remove dead cockroaches from the floor inside the rear dish/prep room and on the small chest freezer located in the basement.

- comments: found an incomplete pest control log book at the time of inspection. Observed no recent inspection receipt (at least twice a year per code) on-site at the time of inspection. The last report, inside log book, is dated 1/26/22. Instructed to have a pest control log book on site with a minimum of two inspection reports per year at all times. Priority foundation 7-38-020(c). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: found boxes of uncooked noodles stored directly on the floor near the washrooms doors. Must elevate off floor and maintain.

40. Personal cleanliness
- comments: observed two food-handling employees preparing food without hair restraints. Management instructed that all food-handling employees must wear a hair restraint while preparing food.

49. Non-food/food contact surfaces clean
- comments: observed heavy grime build up on the sprayer arm in the dish room. Must detail clean sprayer arm and maintain.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed wood planks and other wood stacked along the fence in the dumpster storage area. Instructed to remove and maintain dumpster storage area free of any unnecessary items.

55. Physical facilities installed, maintained & clean
- comments: observed food splatters and grease build on the walls and ceiling in the cook line prep area. Must clean said areas and maintain them.

56. Adequate ventilation & lighting; designated areas used
- comments: observed a burned-out light near the ventilation hood. Must replace light bulbs and provide adequate lighting.

58. Allergen training as required
- comments: observed a food manager without an allergen training certificate. All city of chicago food managers must complete allergen and keep certificates on site. Must provide.
