- comments: observed no paper towels at the exposed hand washing sink in the dish washing area. Management instructed to provide paper towels at all hand washing sinks at all times. Priority foundation violation 7-38-030(c). Citation issued.
22. Proper cold holding temperatures
- comments: observed improper cold holding temperatures of tcs foods inside of the 1-door prep coolers. Observed eggs at 45f, tofu at 51f, pot stickers at 51f, cream sauces at 52f, cooked rice at 50f, and chicken at 44f. Management instructed to maintain all cold tcs foods below 41f. Priority violation 7-38-005. Citation issued.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed the 1-door prep cooler next to the stoves with an air temperature of 55f. The cooler was used to store tcs foods such as tofu and cream based sauces. Management instructed to repair and maintain the cooler below 41f. Priority violation 7-38-005. Citation issued.

- comments: observed the pest control log book not maintained. There are no reports from the past year in the pest control log book. Management instructed to provide at least 2 recent pest control service reports per year in the log book. Priority foundation violation 7-38-020(c). Citation issued.

- comments: observed an infestation of live cockroaches in the food prep area, dining room, dry storage area and in the dish washing room. Approx. 50 live cockroaches were observed throughout the 1st floor of the restaurant. Pest control service is required to eliminate the pest activity throughout. Priority violation 7-38-020(a). Citation issued.

- comments: clean and remove all dead insects from the floor behind the equipment in the prep area, on food storage containers and equipment, on storage shelves, and on wall of the dry storage area and dish washing room.

- comments: detail clean the interior and exterior of the deep fryer, stoves, and wire storage rack in the food prep area. Clean and seal the grease trap in the dish washing area.

- comments: observed no backflow prevention device located at the mop sink in the basement. Management instructed to install.

- comments: seal the holes and gaps in the wall behind the cooking equipment and in the dry storage area. Repair the hole in the ceiling drywall at the corner at the entrance to the food prep area.
