- comments: observed the following tcs items prepared on-site held in the stated locations with no date marking: yogurt and chicken in 2 door prep cooler and lamb and rice in walk-in cooler. Instructed to mark all tcs items with 7 day disposal date. Priority foundation violation 7-38-005. Citation issued.

- comments: observed approximately 20 live roaches in rear prep area in corner behind 3 compartment sink, in and around water heater, and underneath prep table. Instructed to receive additional pest control service and maintain facility free of pests. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed heavy build up of grease and grime on floors and walls throughout rear prep area. Instructed to detail clean and maintain in good condition.
