15. Food separated and protected
- comments: observed containers of uncovered raw chicken stored in walk in cooler on floor. Instructed to cover or wrap all exposed food in walk in cooler to prevent contamination of food.

22. Proper cold holding temperatures
- comments: observed cold tcs food(pooled eggs), held at improper temperatures ranging between 48. 9-51. 8f. Instructed all cold tcs food must be held at 41f or below. Corrected on site manager voluntarily discarded food valued at $50 weighing 3lbs priority violation 7-38-005 citation issued

38. Insects, rodents, & animals not present
- comments: observed opening at base of rear exit outer door. Instructed to seal all outer openings to prevent pest entrance.

39. Contamination prevented during food preparation, storage & display
- comments: observed no left splash guard on handwashing sink in rear prep/dishwashing area. Instructed to install left splash guard on handwashing sink in rear prep/dishwashing area to prevent contamination of prep surfaces to the left of handwashing sink. Observed water from rusty and dusty condensor ceiling vent cover in walk in cooler leaking into container on top shelf. Instructed to relocate food items on shelving underneath leak to prevent contamination.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed rust on shelving units in walk in cooler. Instructed to remove rust from shelving units in walk in cooler, non food contact surfaces shall be constructed of a corrosion-resistant, nonabsorbent and smooth material. Observed boxes and sheet pans stored on floor in rear of facility. Instructed to provide shelving units to elevate all food and non food items in facility at least six inches off of floor for ease of cleanability. Observed containers of raw uncovered chicken stored on floor in walk in cooler. Instructed to provide adequate shelving units to elevate all food and non food items at least six inches off of floor for ease of cleanability.

51. Plumbing installed; proper backflow devices
- comments: observed leaking backflow prevention device at utility sink. Instructed to repair leaking backflow prevention device at utility sink and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed walk in cooler condensor leaking into container. Instructed to repair walk in cooler condensor and maintain. Observed rust on walk in condensor ceiling vent cover. Instructed to remove rust from walk in condensor ceiling vent cover and maintain. Observed missing grout in between floors tiles in dishwashing areas, near three compartment sink and low temperature dish machine. Instructed to smoothly replace missing grout in between floor tiles in dishwashing areas, near three compartment sink and low temperature dish machine for ease of cleanability.

58. Allergen training as required
- comments: at time of inspection facility provided no proof of allergen awareness training certificates for all employees with city of chicago food service sanitation certificates. Instructed to provide proof of allergen training for all employees with city of chicago food service sanitation certificates.
