- comments: observed certified food manager on site with no identification to verify identity. Instructed must have a certified food manager on site with the original city of chicago certificate with identification. Priority foundation 7-38-012, citation issued
22. Proper cold holding temperatures
- comments: observed improper cold holding temperatures of tcs foods. Found 10 containers of: fish fillets, fish nuggets, shrimp, chicken, and gizzards at temperatures ranging 48. 0f-51. 0f stored inside a three door cold holding unit. Instructed all cold holding foods must be 41f or below. Approximately 20lbs at $200 worth of product discarded. Priority violation 7-38-005, citation issued
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed a three door cooler found at improper cold holding temperature. Temperature of unit was 54. 0f at this time. Cooler was tagged at this time. Please reviewed comments for tag removal instructions. Instructed all cold holding units must be 41f or below at all times. Priority 7-38-005, citation issued

- comments: observed a container of dry ingredients not labeled. Instructed to label and maintain.
38. Insects, rodents, & animals not present
- comments: observed dead roaches throughout the facility. Found dead roaches on the floors and walls throughout the front and rear of the facility: surrounding the cold holding units, inside the walk-in cooler, in the office, near the three compartment sink, etc. Instructed to remove all dead roaches, clean, and sanitize all affetced surfaces.
38. Insects, rodents, & animals not present
- comments: observed approximately 4 live roaches and about 10 small flies on site. On the wall above the freezer chest in the front prep area, on the wall near the oil container in the rear dishwashing/ food prep area, and on the wall/pipes/floor near the cooking equipment and hot holding units are stored in the front food prep area. Found small flies on the walls/ boxes near the three compartment sink. Instructed to eliminate live roach and small fly activity. Recommended contacting pest control. Manager killed three live roaches. Priority foundation violation 7-38-020(a), citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed the floors and walls throughout the facility in need of cleaning. Especially under the shelves where the dry ingredients are stored in the rear dishwashing area. Instructed to deep clean and maintain.
