- comments: observed 2 door cooling unit in prep area in need of cleaning. Instructed to remove dried food debris and maintain free of accumulation.

- comments: 6-501. 114 out c 07/29/2026 observed black soiled crates used for elevation throughout basement storage area as well as miscellaneus items stored in corner along side of chest freezers insturcted to remove crates, provide smooth and easily cleanable surfaces to store items at least 6 inches off the floor and remove all unnecesary items. Must maintain premise.
