- comments: observed all necessary items and written procedure not available on site when employees respond to vomitting or diarrheal events. Instructed to add an asterisk to all animal derived foods that can be served undercooked or cooked to order. Instructed to add disclosure to menu stating that asterisked animal derived food is served undercooked or cooked to order. Observed no reminder statement on menu. - comments: observed in use utensils stored in container of water between uses in front of stove. Instructed during pauses in food preparation or dispensing utensils shall be stored on a clean portion of the food preparation table or cooking equipment only if the in use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized throughout the day at least every four hours.

- comments: observed spoons, knives and forks stored laying down in uncovered container on table in dining room. Instructed to store utensils inverted or covered during storage to prevent contamination.

- comments: observed all stoppers in three compartment sink not functioning properly. Instructed to repair or replace all stoppers of three compartment sink so that said stoppers shall function properly in three compartment sink.

- comments: observed door of all gender washroom to not be self closing. Instructed to install self closing device on all gender washroom door so that it shall be tight fitting and self closing.

- comments: observed damaged floor to the right of two door refrigerator in rear storage area of enclosed back porch. Instructed to smoothly repair floor for ease of cleanability to the right of two door refrigerator in rear storage area of enclosed back porch and maintain.

- comments: observed stove with six burners in prep area not completely installed under ventilation system. Instructed to have all stoves with burners installed completely under ventilation system or remove from premises.
