Report date: May 20, 20262. City of chicago food service sanitation certificate
- comments: no city of chicago certified food manager on site or valid documentation on the premises while tcs foods (rice, beef , chicken etc. ) is being handled and prepared. Instructed the person-in-charge that a city of chicago certified food service manager with an original city of chicago sanitation certificate must be on the premises at all times when (tcs foods) are prepared, handled, and/or served. Priority foundation violation 7-38-012, citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the hot water at the women washroom handwashing sink to be at 124. 1f. The water temperature cannot be controlled by the user. Instructed person-in- charge handwashing sink water temperature must be provided of at least 100 f through a mixing valve or combination faucet. (a handwashing sink shall have a maximum hot water temperature of 120 f through a mixing valve or combination faucet). Instructed management to repair and maintain as required. Priority foundation violation 7-38-030(c), citation issued.

- comments: observed chipped paint and rust on the wire storage racks in the walk-in cooler. Instructed to remove chipped paint and rust and repaint or replace the shelves and maintain.

58. Allergen training as required
- comments: observed the certified food manager without the allergen training certificate. Allergen training is required for all employees that have a city of chicago food sanitation certificate. Instructed to complete training and keep certificate on site.
