- comments: observed the pic does not have a valid chicago food santiation certificate. Instructed to obtain and maintain a valid certificate. Priority foundation violation combined with violation #2.
2. City of chicago food service sanitation certificate
- comments: observed no certified food manager (cfm) on site with a valid chicago food sanitation certificate while serving tcs foods (chicken, beef, etc. ). The pic did not have one and the other male employee (j. Howard) did not have his original certificate. Instructed to have a cfm with a valid certificate on site at all times when preparing/serving tcs foods. Priority foundation 7-38-012 citation issued
10. Adequate handwashing sinks properly supplied and accessible
- comments: no hand-washing signage in the women's toilet room. Instructed to provide and maintain a signage for the women's toilet room.
49. Non-food/food contact surfaces clean
- comments: the interior of the fryers with grease and dirt build up. Instructed to clean and maintain the interior of the fryers.
51. Plumbing installed; proper backflow devices
- comments: the base of both faucets on the 3-compartment sink leaks. Instructed to fix the leaks and maintain good plumbing.
58. Allergen training as required
- comments: cfm on site has no allergen training certificate. Instructed to obtain and maintain a valid allergen training certificate.
