8. Hands clean & properly washed
- comments: observed female food handler not properly washing her hands before putting on gloves and handling food. Food handler just went to the front hand-washing sink, wet her hands and was about to shake them dry when she was told to properly wash her hands. She then went to the rear hand-washing sink to properly wash her hands. Instructed to properly wash hands 10-15 seconds using soap, rinse and properly dry hands by using a hand drying device. Priority 7-38-010 citation issued

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand-washing cleanser at the front hand-washing sink. Instructed to provide and maintain at all times. Priority foundation 7-38-030(c) citation issued

- comments: observed no paper towels at the front hand-washing sink. Instructed to provide and maintain paper towels at all hand-washing sinks. Priority foundation 7-38-030(c) citation combined with the above violation

40. Personal cleanliness
- comments: observed food handlers not wearing a hair restraint. Instructed to obtain and maintain wearing a hair restraint when in the food prep area and handling food.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed the front hand-washing sink needs a splash guard to prevent splashing on to the cups/lids stored next to it. Instructed to either provide a splash guard or relocate items. Maintain same.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed the customer's toilet room with debris all over the floors and toilet. Instructed to clean and maintain toilet room.

60. Previous core violation corrected
- comments: observed the core violations from report #2628368, dated 12/11/2025 not corrected and/or maintained. Instructed to correct and maintain: -55. Observed a hole in the floor cover that leads to the floor grease trap located by the 3 compartment sink. At the time of the inspection the facility has a subway cup covering the hole. Instructed to repair or replace the floor cover for grease trap. -58. Observed the manager on duty does not have proof of allergen training. Instructed to meet requirement and provide proof for all city of chicago food service sanitation managers. Priority foundation 7-42-090 citation issued
