38. Insects, rodents, & animals not present
- comments: observed a 1/4" gap on sides and bottom of the rear exit door. Must provide a tight fitting seal at all outer openings to prevent the entry of pests.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed soda crates and pallets being used as shelving throughout the prep and storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.

51. Plumbing installed; proper backflow devices
- comments: observed water in the 3 compartment sink slowly draining. Person in charge was advised to unclog sink as a possible solution. Noted 2 customer toilet room 1st toiletroom handsink slow draining, 2nd toilet room has a leak in draining pipes instructed to correct issue repair and maintain at all times.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed the toilet room in need of cleaning. Instructed to detail clean and maintain at all times.

56. Adequate ventilation & lighting; designated areas used
- comments: observed a burned out light-bulbs in exhaust system. Instructed to replace bulb and maintain lighting in the unit.

60. Previous core violation corrected
- comments: noted continued non-compliance from report # 2623351 conducted on 09/05/25. Previous core violations not corrected: (51)5-204. 12: **c** backflow prevention device, location (no backflow device) observed no backflow prevention device at the utility mop sink located in rear closet room. Must provide a backflow preventor device and maintain. (55) 6-201. 11: **c** floors, walls, and ceilings- cleanability observed dust build up on ceiling ventilation vents inside customers washrooms. Must clean and maintain. Observed dirt, grease and debris on floor under around and behind all heavy cooking equipment, under refrigerations units, prep tables and rear food prep area. Must clean and maintain. Observed dirt and debris on floor drains throughout premises. Must clean and maintain. (55) 6-501. 11: **c** maintained in good repair, facilities (facilities not in good repair) observed opening into wall next to prep tables in rear food preparation area. Must seal opening and maintain. Observed damaged wall tiles behind exposed hand washing sink located in rear food prep area. Must repair/replace and maintain facility in a good repair. (58) 2-102. 13: **c** allergen training - all certified food service sanitation managers employed by a restaurant must receive or obtain training in basic allergen awareness principles within 30 days after employment and every 3 years thereafter. Training programs must be accredited by the american national standards institute or another reputable accreditation agency under the astm international e2659-09 (standard practice for certificate programs). Observed certified food service manager without allergen training certificcate. Instructed above that all city of chicago certifed food service managers must obtain allergen training certificate. Must correct core violations by the date on the report. Priority foundation violation #7-42-090. Citation issued.
