Report date: Jun 8, 202625. Consumer advisory provided for raw/undercooked food
- comments: found in-complete consumer advisory disclosure statements written on the table menus. Instructed to disclose which foods can be ordered "raw" or under cooked and link those foods with an asterisk (*) to a reminder statement that states that eating raw or under cooked foods can increase your risk of food-borne illness. Priority foundation violation 7-38-005. (***see comments).
37. Food properly labeled; original container
- comments: observed bulk containers in rear without a label. Instructed to provide the common name on all bulk containers to properly identify item(salt, sugar, flour etc).
38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on the premises. Observed approximately 20 rodent (mice) droppings on the floor and on the storage rack inside the side dry storage room and approx 10 rodent (mice) droppings at corner storage shelf next to the kitchen 2 door stand-up cooler. Instructed to detail clean and remove all rodent droppings. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a), citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed a torn door gaskets inside of the kitchen 2 door stand-up cooler. Instructed to replace the torn door gaskets and maintain.
55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean floors along the walls and in all corners throughout prep/cooking area especially under and around cooking equipments and the side dry storage room and maintain.
58. Allergen training as required
- comments: observed the certified food manager without the allergen training certificate. Allergen training is required for all employees that have a city of chicago food sanitation certificate. Instructed to complete training and keep certificate on site.
