- comments: observed no certified manager on site at time of inspection while tcs items such as eggs were prepared. Instructed to maintain an employee on site at all times holding a valid city of chicago foodservice manager certificate. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no kit for responding to vomit and diarrhea on site at time of inspection. Instructed to provide and maintain on site at all times. Priority foundation violation 7-38-005. Citation issued.

- comments: observed no handwashing signage at exposed handwashing sink located in front coffee station. Instructed to provide.

- comments: observed heavy build up of ice in interior of prep cooler located in front coffee station used to store milk. Instructed to remove ice and maintain in good condition.

- comments: observed no chemical sanitizer testing devices on site at time of inspection. Instructed to provide sanitizer and test strips and maintain on site at all times. Priority foundation violation 7-38-005. Citation issued.

- comments: observed the following repeat core violation not corrected from report #2617257 dated 5/12/2025: #51 "observed no sink used for dumping liquid waste at front coffee beverage preparation area. Instructed management that a minimum of 1 compartment sink to be installed for obtaining water and discarding liquid waste must be available at the coffee preparation area. " instructed to correct and come into compliance. Priority foundation violation 7-42-090. Citation issued.
