Instructed a city of chicago certified food manager must be on site at all times with the original city of chicago certified food manager certiticate while open and operating. - comments: observed food on site prepared at home. Per person in charge at the time of the inspection food items are sometimes cooked at home and transported to facility. Found ground beef, italian beef, and nacho cheese on site. Instructed all foom items must be prepared on site and never at home, per food code. Priority 7-38-005, citation issued

- comments: observed stove on site with no ventilation system above it. Instructed must install ventilation above stove in rear prep/dishwashing area and maintain.
