- comments: observed no certified manager on site at time of inspection. Instructed to maintain am employee holding a city of chicago foodservice manager certificate on site at all times while open and operating. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no handwashing signage at exposed hand sink located in front customer service area. Instructed to provide.

- comments: observed no internal thermometer in cooler located in front customer service area. Instructed to provide.

- comments: observed approximately 5 live roaches in proofing box and rodent droppings in the following areas: observed approximately 5 rodent droppings in rear storage area underneath lockers, 10 rodent droppings in rear storage area underneath canned goods, and approximately 15 rodent droppings in corner behind bagel kettle. Instructed to clean, receive additional pest control service, and maintain facility free of pests. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed no sink stoppers at 3 compartment sink in rear dishwashing area. Instructed to provide.

- comments: observed no grease trap at 3 compartment sink located in rear dishwashing area. Instructed to install and maintain.
