- comments: 5-203. 11--- noted missing hand washing sink in the dish washing area. Instructed to re-install a hand washing sink with hot and cold running water, soap and paper towels that properly drains wastewater. Priority foundation violation 7-38-030(c) citation issued.

- comments: 3-603. 11--- new menu missing a consumer advisory. Menu does not disclose and inform consumers the specific menu items that are raw or undercooked such as steaks and burgers. Must disclose and remind customers of such food on both the printed and virtual menus. Priority foundation violation 7-38-005 citation issued.

Instructed to replace/repair all to be smooth and cleanable. ----noted melted/burnt plastic food containers must be replaced. : 4-501. 12-- noted clean stored red color coded cutting boards at the dish area in poor repair with deep, blackened grooves. Instructed to replace or re-surface.

- comments: 4-602. 13--- must not use cloth towels as liner for shelving on the cooks line or cardboard as liner in the walk-in cooler. Must be a smooth, cleanable surface. 4-601. 11(b)-- noted food encrusted wall mounted fry slicer. Must clean after each use.

- comments: 6-201. 11--- must not use duct tape as a means to seal/repair on the dish area wall. ----must not use expanding spray foam that is bumpy and uneven as a sealant and unable to clean. Must cut/smooth down to be cleanable. ----broken/missing floor tiles throughout the dish area. Instructed to replace/repair. ----must install a smooth cleanable ceiling above the basement ice machine and box freezers in the basement. ----hole in the ceiling of the basement liquor room storage. Instructed to seal. ----water damaged ceiling tiles in the basement dining room. Instructed to replace. ----floors throughout the walk-in cooler with excessive food debris. Instructed to clean and maintain. 6-201. 13--- missing/damaged baseboard coving in the walk-in cooler with collected food debris. Instructed to replace.

- comments: 6-304. 11--- inoperable mechanical ventilation in the men's basement washroom. Must repair/replace.
