- comments: observed 5 live roaches around utility sink, grease trap, and in bathroom and 3 live flies in bathroom and rear storage area. Instructed remove pests, receive additional pest control service, and maintain facility free of pests. Priority foundation violation 7-38-020(a). No citation issued.

- comments: observed ice scoop stored directly in ice maker. Instructed to store ice scoop outside ice machine.

Must use food grade disposable bags or washable containers. Also noted metal tin cans used as food storage containers inside the cooks line cooler. Must provide washable containers. ' #55 ' observed extremely dirty walls throughout the employee washroom surrounding the toilet and hand sink. Dirty food splattered walls at the three compartment sink, one compartment sink and rear dish hand washing sink and all attached plumbing piping. Must clean all and maintain.
