- comments: observed no handwashing sink signage at front handwashing sink. Instructed to provide handwashing sink signage to all handwashing sinks and maintain.

- comments: observed approximately 50 rodent droppings in areas throughout facility: below toilet in employee restroom, below 3-compartment sink in rear area, below upright coolers in front area. Instructed to remove droppings and clean and sanitize affected area and maintain. Priority foundation violation. 7-38-020(a) citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed bottled beverages stored directly on the floor in the front service area. Instructed to relocate beverages and store at least 6 inches above the floor and maintain.

- comments: observed aluminum foil used to line nonfood-contact shelves in the kitchen area. Instructed to remove and maintain shelves with material that is smooth, non-porous, and easily-cleanable.

- comments: observed soil and debris on interior cavity of coolers throughout kitchen area. Instructed to clean at adequate frequency to prevent buildup of soil and debris and maintain.

- comments: observed hot water knob of rear handwashing sink faucet functional but in poor repair. Instructed to maintain all plumbing fixtures in good repair.

- comments: observed large gap between floor tiles in front service area/kitchen area. Instructed to remove/repair gap and maintain floors that are smooth, non-porous, and easily-cleanable.
