- comments: observed one of the front entrance doors not rodent proofed at the bottom as required; must rodent proof door to avoid possibe rodent entry.
38. Insects, rodents, & animals not present
- comments: observed 70 or more mouse droppings on premises. Found mouse droppings on the floor behind the small ice cream freezer near the front entrance door, and on various shelves in the rear storage room. Instructed to clean all areas and have pest control company service establishment prior to reinspection next week. Priority foundation violation 7-38-020(a) issued.

- comments: observed the faucet at the utility sink leaking when water turned on; instructed to repair leak.
55. Physical facilities installed, maintained & clean
- comments: insructed to clean the floors and storage shelves in the areas where droppings were observed. Instructed to clean the interior of the mop bucket and the utility sink to remove dirt observed. Instructed to remove dust observed on the fan covers inside of the walk-in cooler.
