- comments: observed over 80 mice droppings scattered on floor, electrical outlets, cabinets, shelving units and invoices in prep area, on floor, toy and art supply shelves in classrooms, on floor inside storage closets in classrooms, over 20 rat droppings on floor near refrigerator and under 3-compartment sink in prep area and utility sink in utility closet, several flying insects in washroom, prep, and classroom areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed 1/4' gap on bottom left corners of front north door, rear middle door, and rear south door. Instructed manager to seal all outer openings on doors to prevent pest entry.

- comments: observed leaking drainpipe under rear hand washing sink in infant room. Instructed manager to repair and maintain.

- comments: observed peeling paint on ceiling in classrooms, loose baseboards along walls in prep area, openings in walls and ceiling around pipe fittings in prep, classroom, washroom and storage areas. Instructed manager to repair walls and ceilings in all areas and maintain.
