Report date: Sep 3, 20252. City of chicago food service sanitation certificate
- comments: observed facility without a valid city of chicago food service sanitation certificate posted onsite for food employees. Servesafe certificate for alexandra gonzaleez was presented at time of inspection. Informed person in charge city of chicago original food service certificate must be posted along with the certified manager on site when time and temperature control for safety(tcs) foods are stored. Priority foundation violation #7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handsink in front prep not stocked with hand soap. Informed person in charge that hand soap must be available at handsinks at all times. Pic obtained hand soap for hand sink during time of inspection. Priority foundation violation #7-38-030(c). Citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed a blue panel inside the ice machine that touched ice filled with a black mold-like substance. Instructed manager to wash, clean, sanitize and maintain. Ice is used for human consumption. Pic instructed food handlers to empty and clean and sanitize ice machine. Priority foundation violation. 7-38-005. Citation issued.

- comments: noted internal temperatures of the following time/temperature control for safety food items to be improper: egg patties in prep area at 68. 6f and 66. 9f. Manager immediately and voluntarily disposed of 4 lbs of products worth $180. 00 through the denaturing process. Priority violation #7-38-005. Citation issued.

- comments: observed food containers stored on the floor in the walk-in cooler, and basement storage areas. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: observed scoops for dry storage containers being stored directly inside of the container. Instructed to properly store all utensils between use.

- comments: must invert pots on lower shelves of storage racks in dsih area.

- comments: observed milk crates being used as shelving throughout the prep and storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: observed clutter stored on crates and floor in basement storage area. Instructed manager to remove all unused articles and equipment to prevent pest harborage.
