- comments: noted sliced canadian bacon with brown and green mold and tomatoe sauce stored in tin can with white, blue and black mold inside 2 door refrigerator. Approximately 6 lbs at $23. Person in charge discarded and denatured product. Priority violation 7-40-050(a) citation issued

16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice scoop in need of cleaning and stored on top of meat grinder. Instructed to properly clean, sanitize and air-dry ice scoop storing in proper holder for ice scoop.

- comments: observed black, pink and brown slime substance dripping onto ice cubes stored at below ice bin in rear prep area. Ice is used for human consumption. Instructed to remove ice deep clean and sanitize and maintain. Priority foundation violation #7-38-005. Citation issued

- comments: observed ready-to-eat, time/temperature control for safety (tcs) foods (sliced canadian bacon, nutella and mascapone dessert, tiramisu to indicate the product's name and date in which the food must be sold, discarded or consumed within 7 days of preparation. Instructed to provide proper labels for date marking and discard date for refrigerated, ready-to-eat, tcs foods held over 24hrs placed in all cold-holding units. Person in charge discarded and denatured approx. 4 lbs valued at ____priority foundation violation #7-38-005. Citation issued

- comments: noted no metal stem thermometer on site to monitor the internal food temperature of tcs food cooked, delivered and served. Instructed to obtain and maintain proper thermometers on site at all times. Priority foundation violation #7-38-005. Citation issued

- comments: observed pest control log-book with no service reports; pest control log book has a service request log with dates 4/3/24 and 4/12/24 with no service reports. All retail food establishments must maintain a logbook which shall be kept for a period of twelve (12) months and include a copy of the service tickets and the pest control company�s illinois department of public (idph) health license number. Instructed to provide. Priority foundation violation #7-38-020(c)

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: noted plates stored on drying rack in front of kitchen stove with dust and grease. Instructed to wash, rinse, sanitize and store inverted to prevent contamination of food contact surface.

- comments: noted cooked equipment in kitchen not clean. Instructed to deep clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: noted clutter in front of kitchen prep cooler and in rear storage. Instructed to remove all unnecessary items and elevate kept items on proper shelving that is non pourous, smooth and easy to clean and a minimum of 6 inches from the ground.
