- comments: observed containers of raw chicken stored on floors in walk in cooler. Instructed to elevate onto shelving to prevent any source of contamination.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed dirty cutting boards throughout prep area with dark stains and deep grooves. Must detail clean or replace.
51. Plumbing installed; proper backflow devices
- comments: observed no visible back flow preventer device on ice machine water line located next to hand washing sink in prep area. Must install.
52. Sewage & waste water properly disposed
- comments: observed hose faucet with a tube going into hand washing sink. Pipe was connected to ice machine. Manager stated they use it for filtered water. Instructed manager to remove from hand washing sink.

- comments: observed sewage backup at floor drain under prep table in rear when water is being fully drained at 3-compartment sink. Sewage must be completely drained into an approved sewage system and must be repaired and maintained at all times. Priority violation. 7-38-030(c). Citation issued.

- comments: observed broken ceiling tile in rear by 3-compartment sink. Must repair.

- comments: allergen training is not complete. Employees who obtain the city of chicago food manager certificate must complete allergen training.
