- comments: observed heavy debris on floors at rear stairwell and behind walk in freezer in basement and also near water heaters in basement supplies (to-go containers) storage area. Must clean and maintain floors.

- comments: clutter and unused/unnecessary items and equipment in basement storage area with to-go containers. Must clean and organize, remove items to facilitate cleaning and to prevent pest harborage.
59. Previous priority foundation violation corrected
- comments: previous priority foundation violation not corrected from 4/21/2021, report #2500572. #38- observed approximately 153 mice droppings throughout establishment. 3 mice droppings behind mixer in 1st floor prep area, approximately 75 mice droppings were found in rear stairwell where pots and utensils were being stored and approximately 75 mice droppings found in basement rear storage area where to-go containers and utensils are stored. Additionally found 5 live cockroaches behind reach in freezer in basement food prep area. Must clean and remove all evidence of pest activity on premises. Must have a licensed exterminator provide pest control services on site to correct issue. Priority violation. 7-42-090. Citation issued.
