- comments: observed no hand washing signs at hand sinks in men's and women's staff toilet rooms. Must provide and maintain.

- comments: observed evidence of appx. 30 or more live small flies flying around,on walls and ceilings in dish washing area,mop sink closet,dry food storage area,trash bin and appx. 10 or more live large flies flying around prep,dish washing and staff lounge area. Recommended to have a pest control operator to service premises on or after 7/23/21. (priority foundation 7-38-020(a)(citation issued)

- comments: observed no splash guard in between hand sink and drainboard at dish machine and next to hand sink at prep table. Must provide and maintain.

- comments: observed 3- compartment sink floor drain overflowing and pooling water on floor and slow draining when water is released from 3- compartment sink. Must repair and maintain. (priority foundation 7-38-025)(citation issued)
