- comments: observed no address/location information on prepacked goods sold (cheese and yogurt) in display cooler at rear of store. Distributor name is present. Instructed manager to ensure that all prepacked items are appropriately labeled with name and place of business of manufacturer, packer, or distributor.

- comments: observed 30 mouse droppings under shelves and along walls in the dry storage room next to mop sink and along walls and behind couches in the rear prayer room. Instructed manager to clean, sanitize, and maintain area free of mouse droppings. Priority foundation violation, 7-38-020(a). Citation issued.

- comments: observed food items (boxes of cookies) stored on the floor in the rear dry storage and prayer room area. Instructed manager to elevate at least 6 inches from floor.
